Postmortem Facilitation
After the dust settles you have one question "How do we make sure this never happens again?".
We help your team identify gaps in observability, operations, architecture, and culture — and recover the trust lost when mistakes happen, by showing the team really did learn from the event.
Sound familiar?
“Our website went down and we found out from our customers”
“We found an issue that was going on for months, it was embarrassing, and now I don't know if that was the only one”
“The answers I got from my team after a production issue are making me concerned, I don't think they understand what's at stake here”
Process
01
Debrief
Capture the timeline of the event, and key questions that should be answered while the data is fresh. This doesn't have to be synchronous, but it does need to be fast, a lot of the time the evidence degrades with time.
02
Writeup
We bring a template if you don't have one, but will do guided authoring of the post-mortem including 5-whys and suggested action items. If you have one written, we'll scrutinize it and ask the questions for you, as many times as it takes to get a good document.
03
Readout
We'll facilitate a meeting between stakeholders and the team and mediate the discussion. For example this helps with questions from leadership that the team may not be comfortable handling directly, or raising topics requested in advance that leadership may be concerned with raising themselves.
